Usefulness of Perfusion Imaging in Transient Ischemic Attack Patients without Diffusion Lesions

Abstract
However, because clinical TIA patients frequently show abnormalities on CT or diffusion-weighted image (DWI), it has been claimed TIA with imaging evidence of infarction should be reclassified as stroke. The perfusion-weighted image (PWI) is more sensitive than DWI in detecting acute ischemia, thus the hyperacute transient ischemia may be evident more often on PWI than on DWI. However, few studies have reported the usefulness of PWI in TIA. Here, we report two patients who showed only PWI lesion on initial MRI and delayed appearance of diffusion lesion in the area of initial hypoperfusion.
